Steps New Patients Must Take Prior To The First Visit
To make the most of your time with us, we ask that you please complete the New Patient Registration Forms found HERE and send completed documents to CRMAnewpatients@integramed.com within a week of receiving the documents. This step must happen prior to your nurse work up so that our clinical team can better understand your individual health to give you the best care possible.
Your First Appointment
During this first visit, we will take the time to get to know you and your partner, and you will get to know our physicians, nurses, office staff and financial counselors. We strongly encourage both partners to be present at this initial meeting, although this is not a necessity.
You will meet your physician who will spend 30-60 minutes thoroughly reviewing your medical and fertility history with you. Your physician will also perform a medical evaluation including a fertility assessment and possibly blood work and/or a vaginal ultrasound. (There is no need to fast for potential blood work.) He or she will then diagnose any issues and create a customized treatment plan that will best address your unique situation.
Near the end of your visit, you will meet your nurse who will speak with you in-depth about any recommended diagnostic testing, treatment options and questions you may have.
Maximizing Your Benefits
During this first appointment, many people opt to meet with one of our financial counselors. Our counselors are here to offer you their expertise regarding costs, medical coverage, financing options and payment plans, and answer any questions you may have about the financial aspect of treatment.
We currently accept all insurance policies and participate in most major managed care plans.
