American Society for Reproductive Medicine (ASRM) Guidelines for Elective Single Embryo (eSET) are that you should council every patient who is a good prognosis patient under the age of 38.

Ideally the transfers for Elective Single Embryo are executed on day 3 and day 5 embryo transfers:

  • One to two embryos should be inserted on day 3 embryos
  • One embryo should be inserted on day 5 blastocyst

Elective Single Embryo (eSET) versus Dual Embryo Transfer

The difference is the transfer of one versus two embryos. The problem with transferring two embryos is you still carry a very high twin rate, especially true with day 5 embryos.
